Degenerative disc disease (DDD) refers to the pain and discomfort caused by the gradual breakdown of spinal discs, which often occurs as we age. By age 60, over 90% of people show some signs of disc degeneration.

Types of Degenerative Disc Disease

DDD can affect various parts of the spine:

  • Cervical Degenerative Disc Disease: Affects the neck (C1–C7). Symptoms include neck pain and stiffness, or pain radiating between the shoulder blades.
  • Thoracic Degenerative Disc Disease: Affects the upper and middle back (T1–T12). Symptoms include mid-back pain.
  • Lumbar Degenerative Disc Disease: Affects the lower back (L1–L5). Symptoms may include lower back pain, numbness, tingling, or weakness in the legs.

Symptoms

Symptoms of DDD can vary based on the severity and location of disc damage:

  • Back pain that ranges from mild to severe.
  • Pain radiating to the arms, buttocks, or thighs.
  • Increased pain when sitting or with activities like lifting and bending.
  • Pain relief when standing, walking, or lying down.
  • Pain that can last from days to months.
  • Numbness or tingling in hands or feet.

DDD occurs due to the gradual wear and tear of spinal discs, which act as shock absorbers between vertebrae. Key causes include:

  • Age: Discs lose water content and elasticity over time.
  • Disc Tears: Tears in the disc from activities or injuries.
  • Bony Changes: Formation of bone spurs due to the loss of disc cushioning.

Diagnosis

Diagnosing DDD involves:

  • Medical History and Physical Exam: Assessing symptoms, spine mobility, muscle strength, and sensory changes.
  • Imaging Tests:
  • X-rays: To check bone structure and rule out other conditions.
  • MRI: Detailed images showing disc changes and nerve impacts.
  • CT Scans: Useful for detailed bone images if MRI is not possible.
  • Electromyogram (EMG): Measures electrical activity in muscles and nerves to detect nerve issues.

Treatment

Treatment focuses on relieving symptoms, preventing further degeneration, and strengthening the spine. Options include:

  • Conservative Treatments:
  • Rest: Short periods of rest with modified activities.
  • Ice and Heat: Ice to reduce inflammation, heat to relax muscles.
  • Medications: NSAIDs, topical analgesics, muscle relaxers, and antidepressants.
  • Steroid Injections: To reduce inflammation and pain.
  • Physical Therapy: Includes exercises, massage, ultrasound, and posture training.
  • Exercise: Strengthens back muscles and reduces strain.
  • Surgical Treatments (if conservative treatments fail):
  • Discectomy: Removal of herniated discs or bone spurs to relieve nerve pressure.
  • Spinal Fusion: Fusing vertebrae to stabilize the spine.
  • Disc Replacement: Replacing damaged discs with artificial ones.

Prognosis

DDD is not life-threatening but can be managed effectively with conservative treatments. Untreated, it can lead to severe pain and permanent damage. Regular exercise, a healthy lifestyle, and adherence to treatment plans can significantly improve outcomes.

To manage DDD:

  • Quit Smoking: Smoking speeds up disc degeneration and affects healing.
  • Lose Weight: Reduces pressure on the spine.
  • Support Mental Health: Psychotherapy can help manage chronic pain and related anxiety or depression.
